Form sent out by "Who's Who in America" for its 1938-39 edition, bearing Ade's small printed biographical entry (2" X 2½") from the previous edition affixed in the blank center area. By way of approval, Ade signs this area boldly in pencil, adding "O.K." An unusual mini-biography.
